Susie Zhao is an experienced and dedicated Diabetes Nurse Practitioner and Credentialed Diabetes Educator (CDE), committed to helping individuals live well with diabetes through personalised, evidence-based care.

She has worked across a range of metropolitan hospitals in Melbourne, including Mercy Hospital Werribee, Sunshine Hospital, Joan Kirner Women’s and Children’s Hospital, Footscray Hospital, Royal Melbourne Hospital, Royal Women’s Hospital, and St Vincent’s Hospital Diabetes Technology Research Group.

Susie provides comprehensive care for people living with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and Type 3C diabetes, supporting patients across all stages of their journey.

Her clinical expertise includes:

• Continuous Glucose Monitoring (CGM) and insulin pump therapy( T:slim, Minimed 780G, Ypomed and Omnipod 5)

• Advanced data interpretation and optimisation of diabetes technology

• Individualised medication(oral medication, weekly injectable and insulin) review and titration.

• Practical lifestyle strategies.

Susie is passionate about empowering her patients with the knowledge, compassion ,tools, and confidence to take control of their diabetes, delivering care that is both clinically thorough and genuinely supportive.

As a qualified Diabetes Nurse Practitioner, Susie is able to provide comprehensive care without the need for a GP management plan or EPC referral. You are welcome to self-refer to access her services.
