Myhealth Forest Lake is a GPA Accredited Practice conveniently located in Forest Lake Village.

This brand new, purpose built centre offers the highest standard of medical care with state-of-the-art facilities. We pride ourselves in offering a friendly, convenient, accessible and professional service to all our patients.

We offer a full range of medical services with physiotherapy, pathology and other comprehensive allied health services onsite. For all enquiries, please contact Myhealth Forest Lake.

Dr Fereshteh is a fully Bulk Billing doctor for all patients who currently hold a valid Medicare Card.
All patients under 16 with a valid Medicare Card will be Bulk Billed during the week and on weekends.

Non-Attendance / Cancellation Policy

$30 non-attendance fee applies for late cancellations (less than 2 hours' notice) or no-shows. A $50 fee applies for late cancellations or no-shows in Allied Health.
