We made every effort to stay open and continue delivering quality care to our beloved community but unfortunately we have simply been unable to continue doing so.
If you need to see a GP, please contact our sister clinics at Myhealth Forest Lake, 241 Forest Lake Blvd Forest Lake (Ph: 07 3278 9400).
If you would like your medical records to be sent to your new treating GP, please sign a request form at your new practice & the new practice can email this request to: deltafamilydoctors.reception@myhealth.net.au so that your new treating GP can continue your care.
Thank you for your understanding and we hope to be able to continue to service your healthcare needs at our other Myhealth locations.
