About Myhealth Ashmore
Practice
Myhealth Ashmore Plaza opened in July 2015. Taking over the site of Ashmore Plaza Medical Centre, it still offers the comprehensive and holistic care for which it has been known over the years and we extend a warm welcome to Dr Farhad Azadzamany and his colleagues to the Myhealth family.

At myhealth we give all our patients the choice of doctor and encourage you to make longer appointments if necessary. We request in the event that you need to change or cancel your appointment that you give us 24 hours notice in advance. Walk In Appointments are accepted and triaged on arrival.
Visits
Home visits may be available at the discretion of your treating doctor within a 5km radius of the practice for patients who meet the criteria. Conditions apply. Please speak with reception for further details.

Pricing/Fees
Myhealth Ashmore is a MIXED BILLING practice. A Gap fee of $40 will occur after medicare rebate for most of the GP consultations. Bulk billing may still available for eligible patients with a valid Pensioner Concession Card, Health Care Card, DVA and Children under 16 years with selected GPs of the practice. Please call our reception on 07 5679 5625 for more information.
Fees
For Saturdays, ALL consults are privately billed with a $40 gap fee after medicare rebate (including recalls, results, children and concession card holders).
PLEASE NOTE: Dr Phillip Myers is a fully private billing doctor. Fees will apply for all the patients and reduced gap fee of $30 for concession card holders.
Skin Check Appointment
Private Fees apply for most skin check appointments.
Spot Check $85 and Full Body Skin Check $130 (before medicare rebate).

You can make an appointment with any of our Ashmore Plaza GPs by calling the clinic on 07 5679 5625 or booking online at www.docbook.com.au. Longer appointments are available should you need them, and we also accept walk-in appointments that will be triaged upon arrival.
If you need a home visit and live within a 5 KM radius of Cotlew Street doctors, this can also be arranged with your treating doctor providing you meet the criteria. Please call the clinic and speak with one of our friendly reception team for further details.
Ashmore Plaza GPs Offer A Wide Range Of Medical Services
At Myhealth Ashmore Plaza we treat everybody with respect and empathy, and always put our patients first, that's why we have a range of healthcare practitioners ready to help you and your family with all your medical needs.
Ashmore doctors can help with all sorts of general health problems, from asthma and heart health to mental health and chronic disease management. They can also provide COVID and travel vaccines, and vaccines and health assessments for your children.
Skin check appointments are available, including checks for skin cancer, and minor surgical procedures can be performed at the centre, as can cosmetic medicine. We also have an in-house pathology service with Sullivan Nicolaides Pathology.
Our team of Allied Health Practitioners can offer holistic care and includes a podiatrist, a physiotherapist, a dietitian and an integrative physician.
